Job summary

Terros Health in Mesa, AZ is seeking a Full-Time Health Navigator to support MOUD programs in Federally Qualified Health Centers. You will educate patients, identify barriers to care, and coordinate referrals to primary care, behavioral health, and community services.

You will work with care teams to promote health literacy and empower patients, including underserved communities, across our integrated care model.

Qualifications

• High School Diploma or GED required with 1 year of behavioral health experience.
• Associate’s or Bachelor’s in health education, social work, or related field preferred.
• Bilingual skills are highly desirable but not required.

Responsibilities

• Educate and advocate for patients, addressing barriers to care.
• Coordinate appointments, referrals, and follow-up with primary care, behavioral health, dental, and specialty services.
• Connect patients with social determinants of health resources and community organizations.
• Promote health literacy through individual and group sessions and distribute health materials.
