Job summary

OCHIN is a 100% remote national nonprofit health IT organization seeking a Transformation Consultant to coach clinicians and leaders in optimizing EHR use, improving quality metrics, and strengthening care workflows.

You will map workflows, identify barriers, and partner with HIT vendors to inform needed improvements while supporting remote collaboration and data-driven decision making. Travel may be required.

Qualifications

• Bachelor's degree in healthcare or related field; master's degree strongly preferred.
• 3+ years in healthcare capability building or similar; 4–5 years a plus.
• Excellent written and verbal communication skills; coaching across diverse environments.
• Familiarity with improvement methodologies and data-driven improvement.
• Knowledge of change management and human-centered design; primary care concepts preferred.
• Experience with electronic health records and workflow visualization tools; Epic preferred.

Responsibilities

• Provide health center clinicians and leaders with strategies and tools to strengthen workflows and health outcomes.
• Organize, analyze, and present EHR or performance data to support quality improvement.
• Collaborate with clinicians and leadership to assess current-state workflows and align with new technology capabilities.
• Support deployment of digital health tools to improve care access and outcomes.
• Manage multiple projects related to clinical improvement and coordinate with cross-department teams.
