The role involves direct coaching, data‑driven planning, and guiding adoption of evidence‑based care pathways, both virtually and on-site as needed. Ideal candidates bring 3+ years in healthcare coaching, experienced with Motivational Interviewing and Lean principles, and proficient with remote collaboration tools.

Qualifications

• Bachelor's degree in healthcare or a related field.
• 3+ years supporting health care teams; 4–5 years a plus.
• Strong problem‑solving, independent work style.
• Experience with motivational interviewing, human‑centered design, Model for Improvement or Lean.
• Ability to facilitate meetings and coordinate cross‑disciplinary teams.
• Excellent written and verbal communication with clinicians and leadership.
• Familiarity with remote tools (OneNote, SharePoint, Teams/Zoom) and JIRA is a plus.

Responsibilities

• Provide direct coaching support to clinical teams, in-person and remotely; facilitate peer learning sessions, webinars, office hours, and technical briefings.
• Assist care teams in obtaining and using data to create clinical/operational action plans, drive decision making, and support adoption of evidence‑based care pathways.
• Use adaptive change, motivational interviewing, trauma responsiveness, and quality improvement to identify needs, clarify roles, and guide improvements.
• Support health center teams in adopting technology solutions to drive practice transformation.
• Manage multiple concurrent clinical improvement projects and collaborate with cross‑department teams; maintain stakeholder relationships.
• Identify, document and share best practice insights and actionable recommendations with internal and external stakeholders.
