Job summary

OCHIN seeks a Practice Coach to support health center organizations in achieving clinical, operational, and quality goals. The role involves coaching clinical teams, facilitating learning sessions, and guiding data-driven action plans to adopt evidence-based pathways.

Responsibilities include enabling change using motivational interviewing and human-centered design, coordinating cross‑disciplinary efforts, and promoting technology-enabled transformation. Remote work is supported; travel required.

Qualifications

• Bachelor's degree in healthcare or related field (master’s preferred).
• Minimum of three (3) years supporting health care teams in a similar role; 4–5 years is a plus.
• Ability to anticipate and resolve problems with minimal precedent; strong process understanding.
• Experience guiding clinicians and care teams with practice changes using MI or human-centered design.
• Strong facilitation and meeting design skills for virtual and in-person settings.
• Experience with telecommuting and remote collaboration tools (Teams, Zoom, SharePoint, OneNote).
• Willingness to travel nationally (up to 25–50%).

Responsibilities

• Provide direct coaching to clinical teams in-person and remotely; host sessions and webinars.
• Help care teams use data to create action plans and drive adoption of evidence-based pathways.
• Apply adaptive change strategies to guide roles and improve processes.
• Support adoption of technology solutions to drive practice transformation.
• Manage multiple clinical improvement projects and collaborate across departments.
• Document and share best practices and actionable recommendations.
