Job summary

Primary duties include assembling, monitoring, and filing patient information in the electronic health record in line with HIM policies and HIPAA requirements, with a focus on maintaining the Master Patient Index. The role requires familiarity with release of information, NH Birth Certifications, and electronic submission processes.

Qualifications

• HS Diploma or GED completed or equivalent.
• 1–2 years in HIM or clerical role.
• Familiarity with EMR workflows and privacy regulations (HIPAA).
• Ability to multi-task and maintain accuracy.

Responsibilities

• Maintains the master patient index (MPI) integrity and reconciles discrepancies.
• Notifies HIM Manager and Privacy Officer of incidents indicating potential liability.
• Gathers, scans, and uploads patient records to EMR with quality checks.
• Performs deficiency processes and monitors completion.
• Handles Patient Portal inquiries such as enrollment or password resets.
• Submits electronic birth certifications using NH Vital Records software.
• Retrieves and distributes patient records for various departments.
• Receives new patient packets, logs, notifies departments, and monitors fax inbox.
• Complies with HIPAA, warrants/subpoenas, guardianship requests and disclosures.
• Maintains cooperative relationships with colleagues and handles difficult situations fairly.
