Job summary

Unity Health Care in Washington, DC is seeking a HIMS Clerk to support coordination, tracking, processing, maintenance, and retrieval of patients' medical records and referrals.

Under supervision, you will review reports, liaise with hospitals, and ensure timely follow-up. Candidates should have at least one year of clinical office experience, understanding of medical terminology, and proficiency with MS Office and EMR systems.

Qualifications

Responsibilities

• Screen, track, and maintain referrals and medical records for review.
• Monitor high-priority and urgent referrals.
• Liaise between Unity Health Care, hospitals, and providers.
• Support timely referral follow-up and closure.
• Contact patients to reschedule missed appointments in eCW.
• Fax records to specialty facilities as needed.
• Send notifications to providers about outstanding referrals.
• Document encounters and follow-ups using established processes.
• Send letters to patients when outreach fails.
• Ensure HIPAA compliance and confidentiality.
• Assign consultation reports to providers and file properly.
• Notify providers of reports with alarm words.
• Update patient demographics as required.
• Maintain accurate fax inbox and contacts.
• Assist with population health outreach by phone.
• Review and assign medical record requests to DataFile Technologies.
• Inform supervisor and providers of problems.
• Perform other duties as assigned.
