Job summary

Mosaic Community Health is seeking a Referral Health Information Technician to ensure timely delivery of medical records and processing of system referrals. You will educate clinical staff, coordinate documents, and collaborate with the HIT and Referrals teams to support centralized processing.

The role reports to the Site Coordinator and requires strong attention to detail, effective communication, and knowledge of medical terminology.

Qualifications

• One year of experience in a medical office setting.
• Referral coordination and health record management preferred.
• Experience working with low-income populations.
• Typing 40 WPM or faster.

Responsibilities

• Document coordination: distribute documents and medical records and manage faxes per procedures.
• ROI coordination: process site ROIs and create an Epic encounter.
• Communicate to care teams: educate staff on HIT/Referrals processes and troubleshoot issues.
• Navigate the CIMS website to check referral status and share with care teams.
• Track referral status with centralized referrals team.
