Job summary

Monarch Healthcare Management is seeking a Health Information Manager (HIM) to maintain the integrity, confidentiality, and accessibility of resident health records across our Minnesota facilities.

The role oversees HIPAA compliance, release of information, census data management, admissions coordination, and discharge documentation. Strong knowledge of medical terminology and Medicare/Medicaid payments is required.

Qualifications

• HI Assistant or HUC experience and training in medical records.
• Knowledge of regulations, survey processes, documentation requirements and legal issues.
• Understanding of Medicare and Medicaid payment systems.
• High school diploma or equivalent.
• Knowledge of medical terminology.
• Long-term care or healthcare experience.

Responsibilities

• Ensure HIPAA compliance and protect confidentiality of records.
• Manage release of information and filing/retention of records.
• Maintain census data, case mix changes, and pay status with audit trails.
• Coordinate admissions and ongoing records, audits, and physician signatures.
• Oversee discharge documentation and quality review.
• Lead department and collaborate on HIPAA compliance and QAPI.
