Job summary

Highland Park Rehabilitation and Care Center is seeking a Medical Records Director (HIM) to oversee the accurate, timely, and confidential maintenance of resident health information in accordance with Colorado state regulations and facility policies.

The role supports clinical operations, survey readiness, and the integrity of health information within our skilled nursing facility, coordinating HIPAA-compliant releases and ensuring retention schedules are followed.

Qualifications

• Experience in skilled nursing or long-term care setting preferred.
• Proficiency in EHR systems and Microsoft Office.
• Strong organizational skills and ability to meet regulatory deadlines.
• Excellent verbal and written communication.

Responsibilities

• Maintain, organize, and secure medical records in compliance with CDPH, CMS, and facility policies.
• Review charts for accuracy; follow up with staff to resolve deficiencies within required timeframes.
• Ensure documentation meets Colorado survey readiness standards at all times.
• Coordinate release of health information per HIPAA and state laws.
• Prepare reports for state inspections, surveys, and internal audits.
• Manage record retention and destruction per state schedules and procedures.
• Provide timely health information to interdisciplinary team to support care decisions.
• Participate in QAPI initiatives related to documentation and accuracy.
• Maintain strict confidentiality and comply with privacy regulations.
