Job summary

Sunrise Services, Inc. seeks a Care Coordinator & Navigation Specialist to join our Health Homes Program in Bellingham, WA. You will connect clients with health and social services, working with a multidisciplinary team to improve outcomes and quality of life for high-risk individuals.

In this role, you’ll screen referrals, assign cases, and engage clients by phone and in person. We offer growth opportunities, competitive benefits, and a supportive, mission-driven environment.

Qualifications

• Education in mental health field with relevant experience required.
• Nursing or social work credentials may be required depending on role.
• Experience with behavioral health services is preferred.

Responsibilities

• Work as a member of a multidisciplinary Health Homes Team.
• Assist Program Coordinator to process referrals from Lead Entities.
• Assist in screening and generating referrals.
• Assist in smart assignment of clients to Care Coordinators.
• Engage with clients via telephone and mail to secure appointments.
• Prepare for site visits and audits.
