Qualifications

• Ability to coordinate care and referrals across multiple entities.

Responsibilities

• Work with Health Homes Team to coordinate care.
• Process referrals from Lead Entities and assist in screening/generating referrals.
• Assist in smart assignment of clients to Care Coordinators and schedule appointments.
