Qualifications

• Education/experience: must meet one of the listed criteria.
• Bachelor's degree with 2+ years in mental health field or equivalent.
• Masters level degree or certified MH Counselor with related qualifications.

Responsibilities

• Work as a member of a multidisciplinary Health Homes Team.
• Process referrals from Lead Entities.
• Screen and generate referrals as appropriate.
• Assist in smart assignment of clients to Care Coordinators.
• Engage hard-to-reach clients and arrange in-person contacts.
• Schedule face-to-face appointments for Care Coordinators.
• Coordinate client consents and initial objectives.
