Job summary

Columbia River Mental Health Services in Vancouver, WA, is seeking a Care Coordinator to support individuals with chronic health conditions through coordination of home health, outreach, and health plan execution.

You will build relationships with community partners, arrange same-day services, coordinate transportation, and help participants access essential services to improve health outcomes and reduce hospitalizations.

Qualifications

• 1–2 years of experience in the social service field with mental illness or chemical dependency preferred.
• Strong communication and interpersonal skills.
• Ability to coordinate care and navigate healthcare systems.

Responsibilities

• Provide care coordination and outreach to individuals referred by Health Home or Managed Care providers.
• Assist participants in navigating the healthcare system and completing a Health Action Plan.
• Coordinate and mobilize treating entities and arrange transportation to appointments.
