Sunrise Services, Inc. is seeking a Full-Time Care Coordinator to join our Health Homes Program. Care Coordinators work with clients to connect them with the health and social services they need to live healthier, more fulfilling lives.

Qualifications

• Bachelor's level degree in social work, psychology, social services, human services or related field.
• CMA with an Associate’s degree.
• Experience in mental health field preferred.

Responsabilités

• Case load of 50 clients; meet the client 1 time per month.
• Assess clients and formulate individualized health action plans; monitor and revise as needed.
• Link clients to health and social resources based on needs and preferences.
• Accept, respond to and process referrals from Lead Entities.
• Engage in outreach activities and conduct face-to-face visits with clients.
• Foster communication between providers and care teams.
• Arrange transitional care from inpatient to other settings as needed.
• Report suspected abuse or neglect as a mandated reporter.
• Maintain accurate records and document progress per policies.
• Support health goals to promote recovery and improve functioning.
