South Florida Community Care Network, LLC is seeking a Population Health & Care Coordination Aide in Sunrise, FL. The role supports the Care Coordination team by assisting in enrollee care planning and navigation of services within the Florida Medicaid framework.

Responsibilities include outreach, assessments, and cohort follow-up, with emphasis on timely, person-centered care and collaboration across providers and care teams.

Qualifications

• Minimum two years in care coordination or chronic disease management.
• Experience with Medicaid or health plan environments is preferred.

Responsabilités

• Assist Care Coordinators in managing enrollee care plans and healthcare services.
• Coordinate with providers to ensure enrollees receive timely, appropriate care.
• Facilitate care planning and regular follow-up to meet enrollee needs.
• Conduct outreach to new enrollees and explain benefits and options.
• Complete initial assessments to identify care needs and onboarding steps.
