Qualifications

• Master’s degree in social work, psychology, or health/public administration, or RN with supervisory experience.
• Experience coordinating services for individuals with disabilities, complex trauma, or special needs.
• Strong organizational, communication, and computer skills.

Responsibilities

• Supervise HHCMs delivering Health Home core services.
• Ensure timely documentation (POC, CANS, progress notes) and IT compliance.
• Maintain adherence to regulations and conduct audits.
• Provide clinical guidance on motivational interviewing and self-management.
• Assign and monitor caseloads for service provision and billing accuracy.
• Coordinate with finance on claims and reconciliations.
• Support staffing needs and collaborate with stakeholders to assess member needs.
• Represent the program and coordinate with Medicaid MCOs, SPA, HCBS providers.
• Participate in policy development and quality improvement.
• Oversee hiring, evaluations, and staff performance.
• Attend meetings and trainings; after-hours coverage as needed.
