Job summary

Community Healthcare Network (CHN) in New York is seeking a supervisor to lead the Health Home (HH) teams at an assigned site. You will oversee clinical supervision, staff recruitment and training, and coordinate client services with internal and external partners to drive referrals and enrollment into the Health Home program.

You will manage caseload delegation, perform weekly chart reviews, ensure PHI privacy, and provide guidance through home visits and daily operations using EHR/HIS systems.

Qualifications

• Masters degree in Social Work preferred or Masters level with commensurate clinical training and experience.
• MA/MS Degree in any discipline with a minimum of three years care coordination experience or BA/BS in related fields with four years of care coordination/case management experience.
• Two or more years of supervisory experience.

Responsibilities

• Provides ongoing supervision sessions with the care coordination team.
• Monitor staff schedules and attendance, ensuring appropriate coverage.
• Collaborate with community organizations and internal/external providers to generate referrals.
• Assign clients to care coordination teams and conduct home and field visits.
• Conduct weekly chart reviews and ensure quality service delivery and documentation.
• Operate with EHR/HIS and related systems for daily care coordination operations.
• Perform probationary and annual performance reviews for supervised staff.
• Consult with Deputy Director and Center Director on staffing and procedural needs.
• Identify and interview candidates for vacant positions.
• Attend Managers' meetings monthly.
