Job summary

Community Healthcare Network (CHN) seeks a Health Home Care Manager I to guide chronically ill patients through the health care system, coordinating with a diverse care team and resources. The HHCM will advocate for clients within CHN and with external providers, ensuring comprehensive service coordination and timely reassessments.

The role leads the care team, manages a patient caseload, and documents needs assessments, care plans, referrals, and outcomes while aligning with Health Home

Qualifications

• Bachelor's or Master's degree with 2+ years in care coordination preferred.
• Associate's degree or equivalent with 4+ years in care coordination.

Responsibilities

• Provides direct service to a caseload of approximately 60 patients.
• Provides crisis intervention and health education services as needed.
• Develops individualized patient-centered plans of care with input from other providers and the patient.
• Coordinates patient services with internal and external service providers through regular care conferencing.
• Documents all patient-related encounters and interventions in the chart per workflow.
• Completes annual reassessment in accordance with Health Home and State guidelines.
