Job summary

JCCA is seeking an Health Home Care Manager (HHCM) Supervisor to lead a care management team under Health Home Core Services. The role focuses on addressing barriers to care and ensuring coordinated, member-centered service delivery.

The supervisor will oversee referrals, enrollment, and documentation, guiding staff in clinical and administrative duties, audits, and continuous quality improvement. Travel within NYS may be required.

Qualifications

• Master's Degree in Social Work, Psychology, or Health/Public Administration or RN with supervisory experience.
• Experience coordinating services for individuals with disabilities, complex trauma, or special needs.
• Strong organizational, communication, and computer skills.
• Ability to travel within NYS.
• Confidentiality and adaptability to program needs.

Responsibilities

• Supervise HHCMs delivering Health Home’s six core services by acuity.
• Ensure timely face-to-face contacts and proper documentation.
• Maintain adherence to regulations and conduct audits for quality.
• Provide clinical guidance on motivational interviewing and engagement.
• Assign and monitor caseloads for service provision and billing accuracy.
• Coordinate with finance on claims and reconciliations.
• Support team coverage needs and collaborate with stakeholders.
• Maintain program census, including intake, enrollment, and discharges.
• Attend meetings and trainings, oversee hiring, evaluations, and staff performance.
• Represent the program with Medicaid MCOs, SPA, HCBS providers.
• Participate in policy development and quality improvement.
