Responsibilities

• Provides clinical and administrative supervision to Care Coordinators to monitor performance
• Staff recruitment and hiring
• Develops individualized training plans for staff development
• Tracks staff activities including outreach, intake, assessment, service planning and delivery
• Writes and delivers performance appraisals and conducts quality assurance reviews
• Oversees care provided by the care management team and coordinates with providers
