Responsibilities

• Evaluate members and assess needs, eligibility, and plan approaches.
• Identify high-risk factors and refer to appropriate care management.
• Coordinate and monitor care plan activities and progress.
• Consult with clinicians to improve care quality and outcomes.
• Escalate quality of care issues through proper channels.
• Negotiate options to meet member benefits and healthcare needs.
• Engage members with motivational interviewing to drive healthy choices.
• Provide coaching and information to empower independent decisions.
• Participate in team meetings and mentor colleagues.
• Help members participate with providers in decision-making.
• Document care activities in line with regulatory and company policies.
