Job summary

MHMR of Tarrant County is seeking a Habilitator to provide direct care in nursing homes for adults and children with intellectual and developmental disabilities. The role includes scheduling, transporting clients, and coordinating with health care providers.

You will maintain health documentation, support nursing staff, and travel daily throughout Tarrant County using your own vehicle. High school diploma required; TX license needed.

Qualifications

  • High School Diploma or GED required.
  • Valid Texas Driver's License required.

Responsibilities

  • Provide direct care services in nursing homes for adults and children with intellectual and developmental disabilities.
  • Schedule appointments and transport clients as needed.
  • Communicate with health care providers and staff; maintain health-related documentation.
  • Aide nursing staff and support daily operations.

Are you passionate about making a meaningful impact? Join our team, where your expertise will directly contribute to programs that will make a lasting impact. As a Habilitator you will be providing direct care services in Nursing homes to adult and children with intellectual and developmental disabilities. You will be responsible for scheduling appointments, transporting appointments, communicating with health care providers and their staff, maintaining health related documentation, and aiding with nursing staff. In this position you are required daily travel in one's personal vehicle throughout Tarrant County.

Minimum Requirements

Minimum Education: High School Diploma/GED

License/Certifications: A Valid TX Driver's License
