Job summary

Arizona Department of Economic Security (DES) is seeking a Habilitation Nurse II in Coolidge, AZ, to coordinate skilled nursing care for individuals with I/DD in 24-hour residential settings. The role requires adapting to 24-hour schedules, with weekends, nights, and overtime as needed.

The successful candidate will perform nursing case management, ensure physician orders are renewed, and act as a consultant on treatment plans within AHCCCS contractual requirements.

Qualifications

• Two years of experience working as a registered nurse with individuals with disabilities, in home health, hospice, or long term care.

Responsibilities

• Coordinate and monitor physical health for skilled nursing needs; place vendor calls and referrals.
• Conduct in-person visits to assess needs and collaborate with families; participate in planning meetings.
• Document progress notes; maintain case management records; support individualized needs.
