Qualifications

• Associate's Degree or 2+ years in community health, long term care, and/or chronic diseases of adults.
• 2+ years selling or relationship-building in healthcare or home care settings.
• Exceptional active listening and communication with empathy.

Responsibilities

• Engage, motivate, coach, and educate caregivers via phone and digital tools.
• Assess caregiver and care recipient needs; coach to achieve goals.
• Face-to-face semi-annual meetings with clients, caregivers, and case managers.
• Recruit new Primecare Home Care members and grow the referral network.
• Document coaching activity with focus on quality and outcomes.
