Job summary

Primecare Home Care Services in Indianapolis seeks a Relationship and Growth Specialist to drive client engagement for our Structured Family Caregiving and traditional home care programs. You will build relationships with hospitals, clinics, and case management entities while earning base salary plus commissions and performance bonuses.

Reporting to Sales Manager/Executive Director, you will blend marketing with care coordination, recruiting new members and expanding referrals in the assigned

Qualifications

• Associate's degree or 2+ years in community health, long-term care, or chronic diseases of adults.
• 2+ years in case management, experience with elderly and disabled.
• Strong active listening and empathetic communication.
• Tech-savvy with mobile apps; comfortable teaching others to use them.
• Proficient in Microsoft Office; multitasking in multi-system environments.

Responsibilities

• Engage, motivate, coach, and educate caregivers via phone and digital tools.
• Assess needs; coach caregivers to develop and achieve goals.
• Conduct monthly check-ins with stakeholders to guide eligibility and service entry.
• Hold semi-annual face-to-face meetings with clients, caregivers, and case managers.
• Track referrals and participation to support growth and report on outcomes.
• Research opportunities for external referrals and services.
• Actively recruit new Primecare Home Care members and build brand in the area.
• Communicate with referral sources to spread awareness of services.
• Document coaching activity and provide quality/outcome reports.
• Escalate issues to coaching manager as needed.
• Identify and cultivate at least five new prospects weekly.
• Foster relationships with new referral sources to expand network.
• Allocate four days weekly for active marketing efforts.
• Demonstrate proficiency in cold calling potential clients.
