Job summary

WelbeHealth is seeking Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics to deliver comprehensive, value-based care for Medicare/ Medi-Cal participants. Expect a base salary with quarterly bonuses, relocation assistance, CME reimbursement, and a broad benefits package.

8 patient days with home visits are typical, within a mission-driven IDT. Active unrestricted medical license, ACGME-accredited residency, and board certification preferred.

Qualifications

• Residency program completed by ACGME
• State medical license active and unrestricted
• Board certification preferred
• CDS and DEA registrations required
• CPR/First Aid within 180 days of hire and maintained

Responsibilities

• Perform comprehensive in-person assessments with IDT to develop personalized care plans
• Coordinate discharge for acute, short-term, and long-term placement
• No hospital rounds; typically see about 8 patients per day
