Job summary

WelbeHealth is seeking a compassionate Primary Care Physician (Geriatrics / Family / Internal Medicine) to join a value-based, mission-driven team. Deliver holistic, preventative care with a small panel of frail seniors, lead a multidisciplinary group, and use home visits to meet patients where they are most comfortable.

You'll work a regular, full-time schedule in an on-site clinical setting with strong collaboration, limited patient volume (about 8 per day), and opportunities for professional

Qualifications

• 2 to 5 years of post-residency primary care experience with a focus on geriatrics or chronic disease management.
• Board certification in Internal Medicine, Family Medicine, Geriatric Medicine, or Emergency Medicine is highly preferred.
• Active MD/DO license and ability to enroll Medicare/Medicaid patients.
• CDS certification and DEA registration without restrictions.
• CPR and First Aid certifications within 180 days of hire.

Responsibilities

• Perform in-person holistic assessments of frail elderly patients.
• Lead a multidisciplinary team to develop and review personalized care plans.
• Maintain a low daily patient volume to ensure comprehensive, whole-person care.
• Coordinate care with home visits and community-based settings.
• Oversee discharges and placement needs across care continuum.
• Provide clinical leadership to NPs and PAs within the clinic.
