Qualifications

• 1+ year of experience in caring for older adults.
• Active, unrestricted MD/DO license in California and CPR certification.
• Board Certification in Geriatrics preferred.

Responsibilities

• Provide primary medical care to PACE participants.
• Coordinate whole-person care with IDT members.
• Document all participant interactions within specified timeframes.

Skills

Experience caring for frail and elderly populations

Strong interpersonal and communication skills

Ability to lead independently

Collaboration in a team-based setting

Language skills (English, Spanish preferred)

Education

Graduate of an accredited School of Medicine or Osteopathy

Active, unrestricted MD/DO license in California

Successful completion of an ACGME-accredited residency program

Board Certification in Internal Medicine or Family Practice
