Job summary

WelbeHealth, LLC. seeks Primary Care Physicians in Internal Medicine, Family Medicine, Emergency Medicine or Geriatrics to deliver value-based care through a PACE program.

You will build long-term relationships with participants, often seeing about eight patients daily in a flexible per-diem schedule while contributing to a broader care team. The role emphasizes comprehensive, holistic care and ongoing professional growth, with a compensation package including base pay and bonus, 401(k) match,

Qualifications

• Residency program accredited by the ACGME
• Master's or doctorate degree required
• Board certification preferred
• Active state physician license without restrictions
• State CDS certification and DEA registration
• CPR and First Aid certification within 180 days of hire and maintained thereafter

Responsibilities

• Perform comprehensive in-person assessments with the interdisciplinary team to develop a personalized care plan for each participant
• Coordinate discharge for acute, short-term, and long-term placement
• No hospital rounds – focus on comprehensive care, averaging about 8 patients per day
