Job summary

Edge Recruitment Solutions is seeking a California-licensed MD or DO for a Primary Care Physician role in the Geriatrics (PACE) program in Stockton, CA. The physician will perform in-person comprehensive assessments and coordinate with the IDT to develop individualized care plans for participants.

Under the Medical Director, you will oversee care in nursing homes and hospitals, manage admits and discharges, coordinate with specialists for care coordination, and guide Nurse Practitioners and IDT

Qualifications

• California-licensed MD or DO.
• Board Certification in Internal Medicine or Family Practice.

Responsibilities

• Perform in-person comprehensive assessments and coordinate with the IDT to develop care plans for each participant.
• Oversee all care of participants in nursing homes and hospitals, including admits and discharges.
• Coordinate with medical specialists to ensure effective care coordination.
• Provide direction and support to Nurse Practitioners and other IDT members.
