Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with primary care or internal medicine experience, preferably geriatrics-focused. This role places you in nursing homes or assisted living settings to improve resident outcomes through coordinated, personalized care.

You will collaborate with a supervising physician, facility partners, and case managers to implement Treat in Place protocols in compliance with federal and state regulations.

Qualifications

• APRN or PA license required.
• Degree from an accredited APRN or PA program.
• 3 years' experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.

Responsibilities

• Assess patients' medical and mental health needs and develop a plan of care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patient response and modify treatment plans as needed.
