Job summary

TruHealth is seeking an advanced practice provider (NP or PA) to serve residents in nursing homes and assisted living facilities, delivering personalized care and coordinating with supervisors to prevent hospitalizations.

You will work with facility partners and health plan leadership to implement Treat in Place Protocols in compliance with federal and state regulations. Competitive compensation and growth opportunities are offered.

Qualifications

• APRN or PA license required.
• Minimum 3 years’ clinical nursing or rehab experience in geriatric populations.
• Experience in Fee for Service settings preferred.
• EHR experience and proficiency with Word/Excel/Outlook required.

Responsibilities

• Assess patients’ medical and mental health needs and create plans of care.
• Ensure compliance with local, state, and federal regulations governing clinical services.
• Prescribe medications and order labs, imaging, and referrals as needed.
• Monitor treatment response and adjust care plans accordingly.
