Job summary

TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics experience to join our team serving nursing homes and assisted living communities. You will deliver personalized, coordinated care, improving resident outcomes and coordinating with supervising physicians.

You will exercise independent clinical judgment, collaborate with facility staff and health plan leadership, and implement Treat in Place Protocols in compliance with federal and state regulations.

Qualifications

• License as APRN or PA required.
• Degree from an accredited APRN or PA program.
• 3 years' experience in clinical nursing or rehab in geriatric populations and/or Fee for Service experience.
• Electronic Health Records experience.
• Working knowledge of Microsoft Word, Outlook and Excel.

Responsibilities

• Assess patients' medical and mental health needs and provide a plan of care.
• Ensure compliance with local, state, and federal regulations related to clinical services.
• Prescribe medications and order lab work, diagnostic procedures and consultations.
• Monitor patients' response to treatment and modify plans appropriately.
