Job summary

TruHealth is seeking an advanced practice provider (NP/PA) with primary care or internal medicine experience, preferably geriatrics-focused. You will work primarily in nursing homes or assisted living communities, providing personalized, coordinated health care to Medicare Advantage plan members.

You will exercise independent judgment in treating acute and chronic conditions, consulting with a supervising physician as needed, and collaborating with facility partners and plan leadership to

Qualifications

• Masters or higher (Nurse Practitioner/Physician Assistant) with specialization in geriatrics preferred.
• APRN/NP or PA license required (US).
• Minimum 3 years of clinical nursing or rehab experience in geriatric populations.
• Proficient with Electronic Health Records (EHR).
• Proficient with MS Office applications (Word/Outlook/Excel).

Responsibilities

• Assess patients' medical and mental health needs and create proactive care plans.
• Ensure compliance with local, state, and federal regulations.
• Prescribe medications and order labs, diagnostics, and consultations.
• Monitor patient progress and adjust treatment plans accordingly.
• Collaborate with RN case managers as part of an integrated care team.
