Job summary

TruHealth is hiring an advanced practice provider (NP or PA) to deliver primary care to seniors in long-term care settings. You will work with a team of providers and case managers to improve quality of life and reduce hospital visits.

You will diagnose and treat acute and chronic conditions, collaborate with facility partners, and implement TruHealth’s national model of care in compliance with regulations.

Qualifications

• APRN (FNP/AGNP/AHACNP) or PA license is required.
• Degree from an accredited APRN or PA program.
• 3 years’ experience in clinical nursing or rehab in geriatric populations.
• Experience with Electronic Health Records (EHR).

Responsibilities

• Assess residents' medical and mental health needs and plan proactive care.
• Ensure compliance with local, state, and federal clinical requirements.
• Prescribe medications and order labs, diagnostics and consultations.
• Coordinate with RN case managers as part of an integrated care team.
