Job summary

Qualifications

• MD or DO qualification required.
• Board certification or eligibility in Internal Medicine or Family Medicine.
• Active, unrestricted US medical license is required.
• Active DEA license and US work authorization required.

Responsibilities

• Assess and diagnose patients at Oak Street Health clinics.
• Oversee, direct, and administer primary care delivery.
• Prescribe and manage medications as part of comprehensive care.
• Maintain patient records in EMR systems (Canopy, Greenway).
• Collaborate with regional and central leadership to meet quality goals.
• Work with Practice Managers to direct and manage the center care team.
• Perform other duties as assigned.

Education

Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) Graduate

Internal Medicine or Family Medicine Board Certification (Or board eligible)
