Job summary

Oak Street Health is seeking a Primary Care Physician to deliver equitable, value-based care to local Medicare populations across our neighborhood primary care centers. You will work within a collaborative, multidisciplinary team to focus on quality outcomes rather than volume.

New graduates are encouraged to apply. Some candidates may be considered for a Center Medical Director role as they gain experience and leadership skills. This is a full-time position with a 40-hour week.

Qualifications

• MD/DO degree required and board certification in Internal Medicine or Family Medicine (or eligible).
• Active, unrestricted state medical license required. Board certification or eligibility in Internal Medicine or Family Medicine.
• Active DEA license.
• US work authorization.
• Experience in value-based care is a plus.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and manage pharmaceutical treatments and medications.
• Maintain EHR data and coordinate with care teams.
• Collaborate with leadership to meet health quality goals.
• Work with Practice Managers to direct the center care team.
• Other duties as assigned.
