Oak Street Health in Reading, PA is seeking a Primary Care Physician to deliver equitable, value-based healthcare to local Medicare populations at our neighborhood centers. You will work with a large care team to focus on quality outcomes, preventive care, and an exceptional patient experience.

New graduates are encouraged to apply; experienced physicians may be considered for leadership roles such as Center Medical Director.

Qualifications

• MD or DO required.
• Board certification in Internal Medicine or Family Medicine (or board eligible).
• Active, unrestricted State License.
• Active DEA license.
• Authorized to work in the United States.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee and administer primary care.
• Prescribe and manage medications.
• Maintain patient EMR data.
• Collaborate with leadership to meet quality goals.
• Direct and manage the center care team.
• Other duties as assigned.
