Job summary

Oak Street Health in Reading, PA is seeking a Primary Care Physician to provide equitable, value-based healthcare to local Medicare populations within our neighborhood primary care centers. Qualifications

• MD or DO graduate.
• Board certification in Internal Medicine or Family Medicine (or board eligible).
• Active, unrestricted State License.
• Active DEA license.
• US work authorization.

Responsibilities

• Assess and diagnose patients at local clinics.
• Oversee, direct, and administer primary care.
• Prescribe and administer pharmaceutical treatments and medications.
• Maintain Patient Electronic Medical Record data.
• Collaborate with leadership to meet quality goals.
• Work with Practice Managers to direct the center care team.
• Other duties as assigned.
