Job summary

WelbeHealth is seeking a Primary Care Physician to deliver life-changing, community-based care to our most vulnerable seniors in a team-based setting. You will conduct in-person assessments, partner with the interdisciplinary team to craft personalized care plans, and coordinate timely discharge planning for various placements.

There is no hospital rounding; instead, you will see about eight patients per day, with regular hours and home visits included in the schedule.

Qualifications

• Residency completion is required and must be accredited by ACGME.
• MD/DO degree required.
• Active state physician license without restrictions.
• CDS certification and DEA registration required.
• CPR and First Aid certifications within 180 days of hire are preferred to obtain and maintain.
• Board certification is preferred but not required.

Responsibilities

• Perform comprehensive in-person assessments with the interdisciplinary team to develop individualized care plans for participants.
• Coordinate discharge planning for acute, short-term, and long-term placements.
• Limit patient load to approximately eight patients per day while delivering comprehensive, person-centered care.
