InnovAge seeks a Pharmacist to oversee pharmacotherapy outcomes and ensure compliant medication distribution. You will coordinate with patients, families, and healthcare providers, explaining care plans and medication details, while ensuring proper documentation in the proprietary system.

The position requires a PharmD, active state license, and 2+ years in pharmacy distribution with strong communication and regulatory knowledge. Geriatrics or chronic care exposure is valued.

Qualifications

• ACPE-accredited pharmacy program or foreign equivalent
• Active license/registration with state board of pharmacy
• 2+ years in pharmacy distribution/packaging
• Experience or interest in geriatrics or chronic care management
• Interest in clinical pharmacogenomics preferred
• Strong written and verbal communication with staff, patients, and providers
• Proficiency with pharmacy software and willingness to improve systems
• Problem solving, conflict resolution and project management skills
• Detail oriented with ability to work under pressure and meet deadlines

Responsabilités

• Serve as primary contact for patients, families, and healthcare providers to address MRPs and pharmacotherapy questions
• Ensure accurate, timely, compliant medication distribution and fulfillment
• Plan, implement, and supervise packaging, labeling, and QA procedures for medications
• Review and approve prescription orders and ensure correct dosing, directions, and quantities
• Provide timely pharmacotherapy recommendations to care providers to improve outcomes
• Ensure compliance with state/federal laws and internal policies; address QA, HIPAA, and Fraud Waste & Abuse issues
