Innovative Integrated Health, Inc. seeks a Pharmacy Technician Liaison to accurately manage medication records and educate elderly patients about medications. You will coordinate with pharmacists, nurses, and providers to ensure timely delivery and high patient safety within the center’s pharmacy operations.

You will also handle inventory, SBAR requests, and participate in care coordination meetings, with flexibility to provide remote coverage for other centers as needed.

Qualifications

• Pharmacy Technician certification (CPhT) and CPR/First Aid.
• Experience in healthcare/pharmacy setting preferred.
• Strong attention to detail and data accuracy.
• Ability to work with elderly patients and families.

Responsibilities

• Manage medication records and ensure accuracy in the pharmacy system.
• Educate patients on medications, side effects, and interactions.
• Coordinate with pharmacists, nurses, and providers for timely deliveries.
• Maintain inventory and ensure proper storage of center medications.
• Assist in SBAR requests and respond to medication questions from participants.
• Support care coordination meetings and center initiatives.
