Job summary

CommuniCare Health is seeking an experienced Nurse Practitioner to supervise PHP clinical operations across Maryland facilities. The DCO role requires field presence with 60% in the field and 40% in administration, including staff hiring, training, and performance management.

The candidate will oversee high-quality primary care in a value-based model, including telehealth. Licensure across multiple states for telehealth is preferred; travel to facilities is expected as needed.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program graduate.
• National certification through AANP or ANCC is required.
• Active, unrestricted state license; ability to obtain licensure in additional states for telehealth.
• 2+ years of clinical NP experience in geriatric, adult, or family practice settings.

Responsibilities

• Oversee delivery of high-quality patient care across PHP facilities.
• Provide ongoing performance feedback and coach staff; participate in disciplinary actions when needed.
• Develop and implement operational strategies for clinical teams and ensure regulatory compliance.
• Manage staff resources, hiring, training, and performance evaluations.
• Ensure documentation in EMR is complete and compliant.
