Job summary

Twin Cities Physicians is seeking an Advanced Practice Wound Care Nurse Practitioner (AGNP/CNP) to deliver high-quality geriatric wound care across assisted living, memory care, skilled nursing, and long-term care communities in the Twin Cities Metro area.

This full‑time role combines advanced wound care, geriatric medicine, chronic disease management, and community-based rounding to achieve measurable patient progress and improved healing outcomes.

Qualifications

• Active Minnesota CNP license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing preferred.
• Strong clinical judgment and ability to manage medically complex patients.
• Valid driver's license and ability to travel throughout Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing wound management.
• Conduct routine rounding visits within assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and community staff.
• Support chronic disease management and continuity of care initiatives.
• Help improve healing outcomes while reducing avoidable complications and hospitalizations.
