Job summary

Twin Cities Physicians is seeking a Nurse Practitioner to lead advanced wound care in assisted living and long-term care communities across the Twin Cities Metro. This full-time role combines geriatric medicine, chronic disease management, and community-based rounding to deliver measurable healing.

You will perform wound assessments, treatment planning, and ongoing management; collaborate with physicians, nurses, and therapists; manage medications and follow-up care; and travel across the metro

Qualifications

• Active Minnesota CNP license; AGNP preferred.
• Experience in wound care or geriatrics.
• Willingness to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments and treatment planning.
• Conduct rounding visits in assisted living, memory care, skilled nursing, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, carers, and community staff.
• Support chronic disease management and continuity of care initiatives.
• Help improve healing outcomes and reduce complications.
