Job summary

Habitat Health in Sacramento, CA, is seeking a Part-Time Social Worker (MSW) to support elderly participants within our PACE program. Work under the Social Services Manager as a critical IDT member to ensure assessment alignment and care plan adherence, with small 24-hour weekly schedule on Tue and Thu.

Qualifications

• Master’s degree in Social Work (MSW).
• A minimum of 1 year experience working with the frail or elderly.
• Experience coordinating and facilitating care conference meetings.
• Proof of medical clearance for communicable diseases, including a TB test.
• Proof of all immunizations are up to date.
• Proof of current CPR/BLS certification required.
• Proof of valid CA driver’s license, personal transportation, and auto insurance as required by State law.

Responsibilities

• Contribute to a center experience that Participants want to spend time in, a team culture that cares and creates joy.
• Collaborate as part of the Interdisciplinary Team (IDT) to optimize health status and quality of life.
• Conduct social work assessments to determine psychosocial needs, preferences and goals.
• Deliver and document social work interventions including arranging resources and services.
• Coordinate care transitions and facilitate care conference meetings.
• Provide discharge planning when participants disenroll from the program.
