Job summary

Habitat Health in Sacramento, CA is seeking a compassionate participant care assistant to support daily living and safety for participants in our PACE center and in their homes.

You will help with feeding, mobility, hygiene, and activities, document care in the EMR, and work with a caring team to ensure a respectful, engaging, and safe experience.

Qualifications

• Completion of accredited vocational nursing assistant course or comparable participant care experience.
• High School diploma or GED.
• Knowledge of PACE program.
• Food Handler's Certification required; obtain within 30 days if none.
• Ability to work without close supervision and exercise independent judgment.
• Experience with frail or elderly (minimum 1 year).

Responsibilities

• Attend to daily living and safety needs of participants in the PACE center and at home.
• Assist with activities of daily living such as feeding and ambulation.
• Prepare and plate meals following dietary cards.
• Record participant care data in the EMR accurately.
• Maintain clean, comfortable participant surroundings.
• Provide homemaking tasks per plan of care when needed.
• Accompany participants to appointments and remind about visits.
• Monitor safety and respond to changes in condition.
