Job summary

Esperanza Health Centers is seeking a Social Worker for the PACE program on a per diem/PRN dayshift schedule. The role involves coordinating psychosocial services, conducting assessments, and supporting participants and families to maintain independence at home.

The Social Worker will collaborate with the interdisciplinary team, perform necessary evaluations, and help ensure high-quality, client-centered care in a community-based setting.

Qualifications

• Conducts initial social work assessment and reassessments for all PACE participants.
• Performs comprehensive psychosocial assessments in line with practice act.
• Identifies warning signs of mental health distress and communicates with IDT.
• Maintains rapport and ensures patient satisfaction.
• Coordinates with IDT to develop individualized care plans.

Responsibilities

• Participates in IDT meetings and family conferences.
• Assists in completion of DON documentation as part of enrollment.
• Monitors the quality and effectiveness of services per care plan.
