Job summary

PACE by IIH in Fresno, CA is seeking an Experienced Social Worker with an MSW to join our interdisciplinary team and manage long-term care needs for participants in our community-based PACE program. You will work closely with the IDT to ensure comprehensive, person-centered care plans and timely assessments for elders receiving at-home services.

The role requires strong communication, counseling skills, and the ability to coordinate with family members and community agencies.

Qualifications

• Master’s degree in social work is required.
• Minimum of one year of documented experience with frail elderly population.
• Knowledge of psychosocial, behavioral, and family needs of the elderly.
• Experience in an interdisciplinary team setting is preferred.
• Computer skills are required.

Responsibilities

• Participate as a member of the interdisciplinary team (IDT) and conduct initial and annual assessments.
• Provide education on durable power of attorney and advance health care directives.
• Deliver psychoeducation on depression and cognitive assessment results to participants and families.
• Coordinate discharge planning and liaise with agencies like Social Security, Medicaid, etc.
• Maintain confidential participant records and assist with family meetings and counseling.
