Job summary

PACE by IIH seeks a Social Worker MSW to deliver direct social work case management to participants and collaborate with the IDT to manage long-term care needs.

Responsibilities include assessments, family counseling, discharge planning, and maintaining records in a HIPAA-compliant manner. Requires a Master’s in Social Work and experience with elderly populations, with strong interdisciplinary teamwork and communication skills.

Qualifications

• Master’s degree in social work from an accredited program.
• Minimum of one year of documented experience with frail or elderly populations.
• Strong knowledge of adult protective services requirements and guidelines.
• Ability to work effectively in an interdisciplinary team.

Responsibilities

• Participate as a member of the interdisciplinary team (IDT) and conduct assessments.
• Provide social work case management and education to participants and families.
• Coordinate discharge planning and maintain confidential records.
